Most of them want to stay home as long as possible. This phenomenon requires <strong>more information sharing<\/strong> on a regular basis between the older persons and their professional or informal caregivers (families, carers, nurses, etc).<\/p>\n<p>From 2013 to 2016, the <strong>European project ACCESS<\/strong> has developed an<strong> innovative technological app<\/strong> based on data sharing between the structures involved.
